DOUGLAS B. MARSHALL, JR. FAMILY FOUNDATION GRANT APPLICATION FORM


PLEASE NOTE: This application form has been recently revised and is current as of July 1, 2009. All applications received after July 1, 2009 should be produced in accordance with the revised application form contained on this page.


I. Proposal Summary (one page maximum)

Please provide a one page summary of your grant proposal. Briefly explain the purpose of your organization and, if applicable, the project for which you are asking for funding. Explain why your organization is requesting a grant, what outcome you aim to achieve with the funds, and how you will measure that outcome.


II. Proposal Detail (eight pages maximum)

A. Background (two pages maximum) – Describe the work of your organization, addressing each of the following:

1. History and mission.

2. The need that your organization works to address, and the population that your organization serves. Be specific about how many people your organization serves.

3. Current programs and accomplishments. Please emphasize the achievements of the recent past.

4. Number of paid full-time staff; number of paid part-time staff; approximate number of volunteers.

5. If you are working in partnership with another organization or with several organizations, please give an account of your relationship to these organizations and an explanation of how you work together.


B. Funding Request (four pages maximum) – Please describe the program or part of your operation for which you seek funding

C. Measurement and Reporting (two pages maximum)

1. Please describe how you will measure the success or failure of your efforts. Wherever possible, please explain any quantitative measures you intend to use to in order to gauge the effect on the target population.

2. If your project involves significant measurement and testing, please be clear in your grant proposal how much of the grant would be used in support of that measurement and testing.

3. Please explain how you will report on the success or failure of your efforts. Describe the format, frequency, and duration of reports to the Douglas B. Marshall Jr. Family Foundation.


III. Attachments – Please label all attachments to correspond to the boldfaced, capitalized items below

A. Financial Information

1. Your most recent financial statement, audited. If you are a U.S. based funding organization which will be conveying the grant to an organization in a foreign country, please include financial statements for the foreign organization in addition to those for the U.S. organization (audited whenever possible).

2. Aligned side by side on the same page, your operating budget for the current and prior fiscal year including a list of foundation and corporate supporters and other sources of income, with amounts, for your current and prior fiscal year. If the grant is being used to cover operating expenses, please indicate clearly which expenses the grant will be used for. Again, if you are a U.S. based funding organization conveying a grant to a foreign organization, please include the very same operating budget information for the foreign organization in addition to that for the U.S. organization.

3. Please list the foundations, corporations, and other sources that you are soliciting for funding for this project (or these operating expenses) and, to the best of your knowledge, the status of your proposal with each.

4. If the grant is to be used to fund one or more projects, please give a separate project budget for each one including sources of income and indicating clearly the specific uses of the grant requested.


B. Supporting Materials

1. A copy of your most recent IRS documentation showing your organization’s tax exempt status.

2. Full contact information for those people at your organization with whom we should discuss this grant application.

3. The address of your website(s) or that of other relevant content available online, including any magazine or journal articles you think describe your organization or project particularly well.



We must receive all of the information requested before our Board will consider a grant request.
